CHC Coaching Application & Disclosure Statement * - This form, which must be filled out by all coaches, gives authorization to run a national criminal records check.

Coach Self Evaluation Form * - Using the following form, evaluate how well you carry out your roles as a leader, teacher, and organizer. For each statement, select the word that best describes you. This form can be used to assess you throughout the season.

Roster Stickers - Use this Excel file to print roster stickers.

Tryout Evaluation Form * - Use this form during tryout evaluations.

USA Hockey Coach's Coupon * - This coupon, which is for all USA Hockey Registered Coaches, will go towards the brand new Skills DVD that was recently completed by USA Hockey.

USA Hockey Injury Survey * - One form must be completed for each 'injury'. The definition of an 'injury' is: Any ailment that causes a player to seek medical attention or forces the player to miss a game or practice.

USA Hockey Student Coach Information Form * - Coaches wishing to use student coaches on their team must have the student coach complete this form. Once completed, it must be submitted to the Registrar.
